Use this form to apply for family assistance funding. We will review your application and contact you directly with a decision. Please note that our funds are limited but we will do our best to help meet your needs.

In order to receive assistance, your child must be a currently enrolled Club member. 

Priority will be given to families who are at a complete loss.
